2021-2022 Choice Exit Request Application

In order to withdraw your child or children from a Choice Program school, you must complete and submit this form to the Student Assignment Office for processing and it must be approved. 

If this request is approved, your child or children will be reassigned to the home school district only. No additional transfers will be approved for the remainder of the current academic year.  Please allow at least ten (10) business days for processing. You will be notified of the decision by mail. 

This application does not apply to students transferring out-of-county, out-of-state, or out of the public school system.
